Eligibility Requirements:

U.S. citizen or Permanent Resident (NO Visas or EAD's)

4th year U.S. or international medical student

\ International medical graduate

       • within 5 years of graduation

       • ECFMG certification prior to submission of application

Oral and written English language proficiency

No more than one failure on USMLE/COMLEX Step 1, 2 or CSA exam

U.S. Family Medicine clinic and inpatient experience equivalent to a 4-6 week

      rotation or externship (No observerships or job shadows accepted)

Personal Statement which reflects the applicant’s personal journey and
      commitment to Family Medicine

Official Medical School transcript

Dean’s letter from Medical School

USMLE/COMLEX Steps 1, 2 and CSA score reports by January 15, 2012

ECFMG certificate if international medical graduate

Two letters of recommendation from U.S. physicians (No international letters

      accepted)

       • One letter from a U.S. Family Medicine physician who has directly

       supervised the applicant in a U.S. Family Medicine clinic

       • One letter from a U.S. physician, preferably a Family Medicine

       physician, who has directly supervised the applicant on a U.S.

       inpatient Medicine or Family Medicine service
