Community Health & Outreach Coordinator CHCW
Purpose of the Position: To promote, maintain, and improve individual and community health. Responsible for outreach and education, identification of potential barriers to services, act as a liaison to insure access of services, and be an integral part of development and implementation of the operational plans, policies, and procedures.
Hours: 40 hours per week; M-F
Qualifications: Bachelor's Degree in Nutrition, Health Education, or related field required. Bilingual E/S required
